Emergency Action Plan

‘Site Location’

Reasons to Activate the EAP:

• Any loss of consciousness
• Possible spine injury
• Dislocation, open fracture, displaced closed fracture
• Difficulty or absent breathing/pulse
• Exertional collapse with central nervous system dysfunction
• Uncertainty if you have a medical emergency
  

Emergency Personnel: 

A UCSF AT will be on site for contact sport and playoff events as defined by the SFUSD-UCSF PlaySafe Program contract. For emergencies during activities with no Qualified Healthcare Professional onsite, Emergency Medical Services should be contacted immediately. Once roles are defined, some of the steps should be taken at the same time by different individuals. 

Site Roles & Responsibilities

Emergency Procedures:

• ENSURE THAT THE SCENE IS SAFE

1. 1. 1. Maintain crowd control and keep spectators away from the area of care.

• MAKE THE CALL

• Dial: 9-1-1 or (415) 553-8090

• Tell the dispatcher: My name is ‘Your Name’. My callback number is ‘(###) ### – ####’. There is a medical emergency at ‘Site Location’. The address is ‘555 Franklin St. San Francisco, CA 94102-5207’.

1. 1. 1. 1. Give your exact location and closest cross street. 
      2. Give the following information: name of the patient, type of Injury, status of the patient
         1. Answer any questions from the dispatcher.
      3. Alert the dispatcher of any obstructions, closures, or possible obstacles. 
         1. Grant all access to emergency medical services vehicles.
      4. Send an individual to meet and direct emergency medical services when they arrive.

• Do NOT hang up! Let the dispatcher hang up first!

• EQUIPMENT RETRIEVAL

1. 1. 1. Send someone to get the relevant medical equipment (i.e., AED, rectal thermometer, prepare cold-tub, glucose, first aid supplies, emergency kit).

• CARE FOR THE PATIENT

1. 1. Provide appropriate care that you are trained for (CPR, AED, and first aid) to the patient until emergency medical personnel arrive and take over care. 
   2. Ensure that a trusted adult will accompany the patient to the hospital or follow in a car if not allowed in the ambulance.

Report incident to the school’s Athletic Director and Athletic Trainer immediately! Complete the appropriate injury reporting procedure for the SFUSD.
